Physicians Exam, you will need to: 

1. Schedule a physical exam with your child's doctor.

2. Download the Physicians Exam one for each child that will attend camp.

3. Have your child examined by a doctor and have the Physicians Exam completed.

4. Check the form before leaving the doctor's office to make sure it is completely filled out.

5. Ensure that all medications that you will send with your child to camp are listed on the form.

6. Scan and email it to HealthCenter@cbfny.org
