Tan medical form - pages one and two to be completed by a licensed physician. (exam must be within one year of camp dates) Immunizations must be listed and up-to-date. Those 26 years of age or older need only date of last tetanus booster and ppd testing results.
Pages three and four to be completed by parent, guardian or adult camper. Be sure to fill it in completely, and sign all consents. Include a copy of both sides of the camper's insurance or Benefit (Medicaid) card(s.)
Lime green form - “Acknowledgment of Receipt of Privacy Notice” if we haven’t received one already.
Second Lime Green form - “Acknowledgment of Receipt of Meningococcal Vaccine Information.
Pink CampMeds registration form – must be completed for each camper or guest who takes any prescription or over-the-counter medications, vitamins or supplements, in any form. Then, original prescriptions must be sent to camp at least 3 weeks before camp. If any CampMeds forms are late, they must be accompanied by a $25 late fee, so that we can process them with CampMeds. The camp is paying for the packaging and processing fee for CampMeds, but we cannot be responsible for the late fees. If the camper who takes medications is not registered by CampMeds, his/her application may be canceled. (only exceptions are arrangements for extenuating circumstances.
